Summer Pre-College Health Forms

Congratulations! Please read the following information carefully with regard to finalizing your enrollment in the Marist Summer Pre-College program.

Marist College oversees the Summer Pre-College Health requirements process, through which you will provide information required to comply with state and federal laws, meet public health guidelines, and support optimal health care for you during your time at Marist.  The information provided, as well as all health care you receive while at Marist, is confidential. 

Required Health Forms

Please note that this process involves completion of a number of forms and documentation of immunizations. Please review what is required to fill out each form in its entirety prior to starting each section. In order to be compliant, you must complete all 5 forms.

Before you begin filling out this form, you will need:

• Copies of a physical examination from your health care provider. Please have them in a digital file to be attached to the end of the form.  Preferred file formats are PDF or JPG. 

Before you begin filling out this form, you will need:

• Copy of latest immunization (vaccination) record from your health care provider.  Please have it on hand to report dates as well as a digital file to be attached at the end of the form.  Preferred file formats are PDF or JPG.

Before you begin filling out this form, you will need:

• Names and contact information for at least 3 emergency contacts that can make decisions regarding medical emergencies and behavioral

Before you begin filling out this form you will need:

• A list of all known allergies (food, medication, environmental) and the proper medical procedure/treatments

Before you begin filling out this form you will need:

• Current health insurance card

Please note that your health forms will be reviewed by a medical professional in May after they have been submitted. If there is incomplete or missing information, you will be contacted by Marist later this spring. Thank you!
