In order to complete this supplemental application, please have the following information available:

  • W-2s for all members of the household
  • 1099s for all members of the household
  • Most current income information for all members of the household
  • Bank statements and/or other records of investments
  • Records of any untaxed income (examples include Social Security, TANF, Veteran鈥檚 benefits, welfare assistance, child support, etc.)
